After selecting a form, or multiple forms, and clicking the Add to eOPF Queue button, a pop-up will display. This pop-up displays the forms selected as well as the Folder Side, Form Type, and Effective Date. This information is used by the eOPF system to store the forms in the new hire's eOPF. The Folder Side and Form Type designations are configured for your agency and will automatically populate.
You must enter the Effective Date for each form. If you select more than one form, a date field will display at the bottom of the pop-up allowing you to enter the same effective date for multiple forms. Once the date is entered, click Apply to assign the Effective Date to the selected forms.
After entering the Effective Date, you will click Send to eOPF to queue the forms for transmission.
Note: To close the pop-up without saving the dates or transmitting the forms, click Close.
